Under the direction of the home support and community services leadership team, the incumbent provides high-level strategic advice on home care practices. Acting as a provincial subject-matter resource, this person works with health organizations, government partners and stakeholders from clinical, administrative, legal, ethical and psychosocial areas.
Using strong analytical judgment and a broad system view, the advisor helps implement sector priorities and supports ongoing improvement, standardization and modernization of home care practices across the province. The role focuses on aligning approaches, improving data use and strengthening consistency, quality, equity and overall service performance.
This position also plays an important role in reviewing data, identifying system-wide issues and supporting strategic decision-making. It serves as a link between different functions in the network to promote coherent clinical, administrative and information practices, while helping teams make better use of data to support organizational priorities.
The advisor leads complex cross-functional mandates that require coordination, engagement and change management skills. They plan and support initiatives involving multiple teams and external partners, considering clinical, operational, human, financial and technological factors.
The role includes contributing to performance, quality and equity indicators, as well as producing integrated analyses to support decisions at different levels of the network. Key responsibilities include monitoring home care performance, developing provincial operational guidelines, supporting organizations facing challenges, identifying gaps between institutions and helping explain the causes of variation in practice.
