We are seeking a rehabilitation professional to support individuals and families in Quebec through coordinated service planning and case support. This remote role involves limited travel within the region as needed.
In this position, you will assess client needs, determine appropriate rehabilitation supports, and help develop practical plans that are tailored to each person’s goals and circumstances. You will work closely with clients, family members or caregivers, and other service providers to create coordinated plans that address health, psychosocial, and employment-related needs. A key part of the role is bringing together different services, identifying barriers to progress, and supporting participants in finding community resources and other suitable options.
You will also monitor risk factors that may affect outcomes and take a proactive approach to improving results. The work requires strong judgment, collaboration, and the ability to manage services in a way that is safe, effective, and aligned with program expectations.
A bachelor’s degree in a relevant field is required, along with recognized professional credentials or registration as applicable in Quebec. Candidates should have at least three years of experience in rehabilitation or a related direct service setting and be comfortable working with people facing physical or mental health challenges. French-English bilingualism is preferred, and federal security clearance will be required. Competitive salary and benefits are offered.
