Nurse Case Manager

We are seeking an experienced nurse case manager to support older adults receiving care in the community and in residential settings. This role focuses on intake conversations, in-home and wellness assessments, care planning, and ongoing communication with clients and their families to help ensure needs are understood and service expectations are met. The right candidate will bring strong clinical judgment, professionalism, and a compassionate approach to working with seniors. You should be comfortable using a person-centered care model that encourages independence while still providing appropriate support. The position also requires solid problem-solving abilities, effective conflict management, and the ability to handle sensitive situations with tact and confidence. Responsibilities include assessing client needs, coordinating with families, maintaining accurate records, and contributing to a high standard of care delivery. Strong communication skills, computer proficiency, and experience in case management, community nursing, or a related supervisory role are important. A registered practical nurse or registered nurse designation is required, along with a valid driver’s license, a clean driving record, and the ability to pass background screening.
