We are seeking a Housing Stability Case Manager to provide essential support in homelessness prevention and shelter diversion for individuals in need. This full-time contract position is based in a community setting, with responsibilities including mobile outreach, housing stabilization planning, and coordination of emergency assistance. The role involves connecting clients to crisis services, mental health support, developmental services, and other community resources to ensure sustainable housing outcomes.
Candidates should possess a Bachelor’s degree in Social Work or a related field, along with a minimum of three years of relevant experience in community outreach, housing stabilization, or intensive case management. Strong knowledge of mental health, developmental disabilities, and the effects of trauma and poverty is essential. Excellent advocacy, service navigation, and communication skills are required, including the ability to manage conflict and maintain accurate documentation.
The position involves travel across the designated service area and may require working in various community settings. A valid driver's license and access to reliable transportation are necessary. The successful applicant will be required to undergo a police clearance process.
