This term position leads a range of community wellness and care services, including home support, support for Elders, client navigation, and related health programming. Working with the wellness leadership team, the manager helps design, implement, and improve services that respond to complex needs such as mobility challenges, chronic conditions, aging-related support, accessibility barriers, and acute care situations. The role also involves overseeing high-priority case coordination, making sure service plans are organized, responsive, and aligned with internal procedures and broader care pathways.
The successful candidate will contribute to program planning, funding proposals, budget oversight, and community outreach, while maintaining working relationships with service providers, partner organizations, and external agencies. This position supervises staff, supports coaching and performance management, and helps ensure services are delivered in a respectful, culturally safe, trauma-informed, and person-centered way.
Applicants should have a relevant post-secondary degree or an equivalent combination of education and experience, along with several years in community-based health, wellness, or human services management. Strong knowledge of care coordination, chronic disease support, aging-in-place approaches, and system navigation is important, as is experience working with Indigenous communities and multidisciplinary teams. Experience with financial management, conflict resolution, and restorative or community-based problem-solving will be considered an asset.
