We are seeking a Field Case Manager to join our team in Saskatchewan. In this role, you will serve as a vital link between patients, healthcare providers, and specialty pharmacies, ensuring that patients have timely access to prescribed therapies. The Field Case Manager will provide personalized support to patients, educate clinics on Patient Support Program (PSP) processes, and facilitate key program milestones, including reimbursement navigation and adherence support. Key responsibilities include acting as the primary point of contact for assigned clinics and patients, conducting both face-to-face and virtual visits, facilitating prior authorization submissions, and providing adherence support through proactive follow-ups. You will also assist patients in navigating specialty pharmacy processes and identify barriers to patient access, escalating complex cases as necessary. Accurate documentation of all interactions and case activities is essential. The ideal candidate will have a minimum of 3 years of experience in a Case Manager or Field Case Manager role within a PSP environment, with a background in Nursing or a relevant healthcare discipline. Strong understanding of reimbursement processes and specialty pharmacy workflows is required. Excellent interpersonal and communication skills, along with proficiency in CRM platforms, are essential for success in this position. A valid driver’s license and willingness to travel within the assigned territory are necessary.
