Case Manager

We are seeking a contract Case Manager to provide crisis-oriented support, case management, and rehabilitation-focused services to adults in a community setting. The successful candidate will assess needs, develop service plans, coordinate supports, and deliver practical interventions that help people build skills for daily living, work, learning, and community participation. This role requires strong clinical judgment, the ability to work within a multidisciplinary team, and comfort supporting clients with complex needs through both short-term intervention and ongoing follow-up. The position involves building productive relationships with community partners, housing providers, and other service agencies, while helping clients access resources and maintain stability. Experience in supportive counselling, crisis response, or related human services work is preferred. A degree in a relevant human services or health discipline is typically required, though a combination of education and experience may be considered. Candidates should bring strong communication, organization, and interpersonal skills, along with the ability to work independently in a rural service environment. Familiarity with tenant matters is an asset. Bilingual French/English skills are considered an advantage. A valid driver’s licence, reliable transportation, and willingness to travel are required.
