Job Title: Case Manager Job Type: Temporary Full-Time (6-Month Contract, 35 Hours/Week, Monday to Friday) Language: English required The Case Manager provides community-based support to individuals experiencing severe and persistent mental health challenges. Responsibilities include assessing client needs, developing personalized service plans, and referring clients to appropriate community resources. The role involves assistance across various life domains, including basic needs, daily living, health, housing, finances, social support, legal matters, education, and vocational training. Qualifications: - Bachelor’s degree in Social or Health Services, or a related field, or equivalent experience - Minimum of three years of experience working with adults with psychiatric disabilities - Training in Suicide Prevention and Non-Violent Crisis Intervention - Knowledge of community mental health resources, referral procedures, and psycho-social rehabilitation principles - Understanding of concurrent disorders and challenges related to homelessness The ideal candidate will establish rapport with diverse individuals, support client independence, utilize a client-centered approach, advocate for individual needs, and coordinate service delivery.
