A contract position is available for a Case Manager within a Mobile Crisis Response Team, focusing on providing culturally safe and trauma-informed crisis support to individuals, families, and communities. This role involves collaboration with law enforcement to address various crisis situations, including emotional distress, substance use, and family conflict. The Case Manager will conduct crisis assessments, de-escalation, safety planning, and referrals, ensuring a client-centered approach that respects cultural identities and community connections.
Key responsibilities include responding to crisis calls, engaging with individuals in distress, and providing follow-up support. The successful candidate will work to divert individuals from unnecessary hospital or police involvement when safe to do so, and will also assist in emergency department assessments and discharge planning. Building strong relationships with Indigenous communities, organizations, and various service providers is essential.
Candidates should possess a relevant degree or diploma in social services or a related field, along with experience in crisis intervention and support. Strong communication skills and an understanding of the impacts of historical and systemic issues on Indigenous populations are essential.
