The Case Manager is responsible for case management, psychosocial rehabilitation, crisis intervention, and treatment for individuals facing various challenges. This role requires clinical skills and experience to assess, plan, develop, coordinate, and provide rehabilitative and supportive services within a multidisciplinary environment. The Case Manager will help clients develop new skills to enhance their daily living, learning, working, and social experiences with minimal professional intervention.
Responsibilities include providing telephone-based support, mobile outreach response, and in-person interactions beyond traditional office hours. The Case Manager will manage referrals efficiently and offer immediate information to clients accessing the program. Participation in a 24/7 on-call service is required to ensure continuous support.
Education and Qualifications:
A Bachelor’s degree in a relevant human service or health science discipline is required, or an equivalent combination of education and experience may be considered. One year of experience working with individuals facing various challenges is preferred, along with experience in supportive counseling, case management, and crisis intervention. Knowledge of clinical case management principles and community resources is essential.
A valid driver's license and reliable transportation are required, along with a willingness to work shifts, including evenings and weekends. Bilingual candidates are encouraged to apply.
Salary: $29.85 to $39.09 per hour, based on qualifications and experience. Benefits include enrollment in a retirement plan. Applications should be submitted by the specified deadline.
